Health Canada determined that this product is classified as a Class 3B handheld portable laser, which presents an increased risk of permanent eye injury from direct exposure to the laser beam, even from brief unintentional contact. The Relay Pro Thoracic Stent-Graft System manufactured by Bolton Medical has been recalled due to a defect where the graft fails to unclasp from the delivery system because the proximal clasp becomes disconnected from the outer control tube. This malfunction prevents the stent from being released and cannot be recaptured during the procedure, and has resulted in 4 complaints including 3 deaths, 1 aortic perforation, and 2 conversions to open surgery.
